Referral form and Agreement
Once we receive this form we will commence onboarding the user onto the Cliq Connection app. The user will then receive an email with their login details. The participant will need to download the app from the Apple Store or Google Play Store. The Cliq Connection is funded through a NDIS plan and charged at the following rate:
$85 Registration fee- to set up your profile (1.5hrs @ $42.50) +
3 mth membership $45 per month
6 mth membership $35 per month
12 mth membership $25 per month
